Job Requirements:
· Doctor of Medicine (M.D.) or Doctor of Osteopathic Medicine (D.O.) degree
· Graduate of an Accredited Nephrology Training Program
· Board Certification in Internal Medicine
· Board Eligible or Board Certified in Nephrology
· Ability to obtain medical license in Indiana
